Corewell Health at Home brings home care professionals and advanced technology into patient homes, where they can heal in familiar, comfortable surroundings. Our care team provides primary care, nursing care, rehabilitation, chronic disease management, infusion, medication management, telehealth home monitoring, respite care, hospice and so much more. Our care team will work closely with you and your patients’ family members to ensure that all needs are met. We work with patients who are recovering from surgery, managing a serious illness or have family members looking for respite care. We offer a comprehensive range of programs and services and are available 24/7.

Visiting Nurse Association

Visiting Nurse Association has been West Michigan’s trusted provider of home care services for more than 120 years, and is a licensed home care provider and accredited by the Community Health Accreditation Program. We provide a number of specialized programs, including: 

    • 3-lead electrocardiogram to provide the patient’s physician with real-time data for monitoring
    • Better balance to prevent falls and build balance for patients with orthopedic conditions, vestibular disorders or other balance-related disorders
    • Cancer rehabilitation that helps patients manage the side effects of oncology treatments
    • Heart care for patients recovering from heart surgery or congestive heart failure or who have a left ventricular assist device or other cardiac conditions
    • Home telemonitoring provides daily monitoring of weight, blood pressure, heart rate and oxygen levels for early intervention to avoid visits to emergency department
    • Intravenous therapy that includes antibiotics and total parenteral nutrition
    • Neurologic care for patients recovering from a stroke or neurological condition
    • Orthopedic care provides physical and occupational therapy for musculoskeletal injuries of the spine and joints as well as post-surgical conditions
    • Pediatric care for patients who need physical, occupational or speech therapy or medical nursing services
    • Rehabilitation for patients recovering from surgery or injury who require physical, occupational and/or speech therapy
    • Wound care for chronic or slow-healing wounds to prevent infection and loss of function and activity level

    Private Duty Care

    Private duty care provides both medical and personal private pay services for patients without home care insurance. In addition to medical services such as nursing, rehabilitation, medication management, post-surgical care, dementia care and chronic disease management, we also provide a range of personal services, including:

    Neuro Rehabilitation

    Under the direction of physicians, our home care team includes rehabilitation nurse specialists, registered nurses, licensed practical nurses, nursing assistants and home health assistants who understand the complexities of traumatic injuries. Members of the rehabilitation team include occupational and physical therapists, speech and language therapists, social workers, recreational therapists, music therapists, rehab technicians, therapy assistants and case managers. Care is streamlined and coordinated to ensure optimal results. For adults and children with brain, spinal cord or orthopedic injuries, we off er services ranging from one-time evaluations to intense, multidisciplinary rehabilitation. Spectrum Health Neuro Rehabilitation Services is accredited by the Commission on Accreditation of Rehabilitation Facilities with specialty recognition in brain injury. Our services include:

    Home Based Primary Care

    Traveling to doctor appointments can be difficult for some patients. Home Based Primary Care brings the care your patients need to their homes. Our interdisciplinary team provides long-term, coordinated medical care that includes primary care visits, lab work, care management and social work. Each patient is assigned one physician as his or her main provider for continuity of care. Services include:
