What’s in my medical records?

Your Corewell Health medical records may include information about your:

  • Age, race and other demographics
  • Health history
  • Hospitalizations
  • Immunizations
  • Physician and advanced practice provider visits
  • Prescription medications
  • Tests and treatments, including surgeries

Electronic health records

Corewell Health uses electronic health records. That means all your specialists have access to your health information—so you get coordinated care.

You have a right to see and get copies of your medical records.

Access your Corewell Health medical records 24 hours a day, seven days a week through MyChart, our secure, online patient portal. There’s no fee for this service.

Request paper copies or digital files of your Corewell Health medical records by calling health information management. Call 616.391.1660 between 8:30 am and 5 pm Monday through Friday.

Please have the following information ready when you make your request:

  • Dates of treatment or service
  • Location of treatment or service
  • What information you want (for example, imaging test results, blood test results, etc.)

Once your records are ready, you may choose to pick them up from our office or have them mailed to you. You’ll pay a small fee for paper records or digital files saved to a CD. You may pay over the phone by credit card.

Forward your medical records

Let us know if you need to share your Corewell Health medical records with a provider at another health care organization. We’ll send your records at no cost. You’ll need to fill out a release of medical information form first.

How to update your medical records

To request changes or corrections to your medical record, contact your doctor’s office.

Birth certificates and death certificates

Visit the Michigan Department of Health and Human Services for information about ordering birth certificates or death certificates.
